Antiphospholipid syndrome (APS) is one of the most clinically important acquired thrombophilic conditions in obstetric practice. It is characterized by the persistent presence of antiphospholipid antibodies and is associated with venous and arterial thrombosis, recurrent pregnancy loss, placental insufficiency, fetal growth restriction, preeclampsia, and preterm birth. Pregnancy itself is a physiological hypercoagulable state; therefore, the presence of APS can shift the balance of hemostasis toward pathological thrombosis and microcirculatory placental injury. The relationship between APS and hemostatic disorders during pregnancy is based on endothelial dysfunction, platelet activation, complement activation, impaired trophoblast invasion, and disturbance of uteroplacental blood flow. Timely detection of antiphospholipid antibodies and assessment of coagulation parameters are essential for risk stratification and prevention of obstetric complications. This thesis discusses the pathogenetic mechanisms linking APS with hemostatic imbalance during pregnancy, its clinical manifestations, diagnostic approaches, and preventive strategies aimed at improving maternal and perinatal outcomes.
